M60 cook off question

Does a cook off occur in an m60 only if a unfired round gets stuck in the chamber after sustained fire or can you have a cook off with a round that is ready to fire that is feed chute?

I would think the latter could be very dangerous.

The next round to feed would never get hot enough.

Yeah…the round would have to be INSIDE the chamber of a hot barrel to cook off. Axe me how I know…
